Students under 18 years of age attending Ontario schools are required to
provide proof of immunization against diphtheria, tetanus, polio,
measles, mumps and rubella as outlined in Immunization of School Pupils
Act, R.S.O. 1990, c.11. Exemptions may be granted for medical reasons or
statement of conscience or religious belief, using the special forms
available for this purpose from the Health Department.
When a student receives a booster or immunization, it is the parent's responsibility to pass this information on to the Region of Durham Health Department so that the student's immunization record can be updated. Physicians do not pass this information on to the Health Department. Immunization records are not kept on file at the school; these records are collected and forwarded to the Health Department.
